Report on-the-job injuries as soon as possible

Please remember to report on-the-job injuries to your supervisor and your safety office as soon as possible. Effective mishap reporting allows employers to take immediate action and implement preventive measures to mitigate risks. When mishaps go unreported, they can escalate into more significant issues, potentially causing more harm to individuals and property.


Department of Defense (DoD) components have differing mishap reporting requirements.

− Army: Mishap reporting is required by AR 385-10 and DA PAM 385-40.

− Navy/Marine Corps: the regulatory driver for mishap reporting is OPNAVINST 5102.1 (series) / MCO 5100.29 (series) and OPNAVINT 3750.6 (series). Mishap reports are input in ESAMS.

Air/Space Force: the regulatory driver for mishap reporting is AFPD 91-212 Safety Programs, and DAFI 91-204 Safety Investigations and Reports, and AFI 91-202 US Air Force Mishap Prevention Program. Mishap reports are input into AFSAS.

− Contractors: Report all mishaps to the Contracting Officer’s Representative (COR).
