Information from U.S. Army Health Clinic Stuttgart
U.S. Army Health Clinic Stuttgart has scheduled special physical examination sessions for school-aged children and youth (ages 4-18) participating in school and CYS SKIES activities. Please note the key details and requirements to ensure a smooth process:
Eligibility and Dates:
- TRICARE Prime beneficiaries assigned to the Stuttgart clinic: Must schedule an appointment with your Primary Care Manager to complete your physical.
- All other ID cardholders and dependents: Appointments are available without prior scheduling on the following dates, between 1-4 p.m.:
- July 12
- July 26
- August 9
- August 23
Appointment Booking:
- To book your appointment, please contact the Appointment Line at CIV: (0)6371-9464-2900 or DSN: (314) 590-2900.
- In order to schedule an appointment, all patients need to be registered with the clinic. Civilians and Contractors can do this by walking into the Patient Administration section during normal operating hours. The Sponsor will need to be registered and provide Social Security Numbers and insurance information for each family member being registered.
Location:
- The check-ups will take place at the Army Health Clinic located on Patch Barracks.
Preparation for Your Visit:
- Ensure to bring glasses if used, up-to-date vaccination records (including the German Yellow Card if available), and the first page of AE Form 608-10-1A (Child, Youth, and School Services Health Assessment/Sports Physical).
Special Instructions for Non-TRICARE Prime Patients:
- No immunizations will be provided during these sessions. If required, such as for a CYS Services SNAP Allergy Medical Action Plan, these must be arranged through your respective Host Nation Primary Care Provider who normally prescribes rescue medications.
